When some approvals are "rejected" The status column shows as "Rejected" on other requests it shows as "completed" in the status column. If yes, the Approve or Reject options you set in Form design are different from the Responses option settings in Workflow settings in Teams Approval and they aren't linked and related. The Status column's value is based one the options you defined from the Responses option settings.

So to get rid of the confusion, I suggest you remove the Approve or Reject options you set in Form design and just use the defined options in Workflow settings in Teams Approval to complete the flow process like the following.
